Appointments and Cancellations
We ask that you provide at least 24 hoursâ notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient in need.
- Cancellations made with less than 24 hoursâ notice or missed appointments (âno-showsâ) will result in a $75 fee.
- Appointments are typically 30 minutes for medical marijuana evaluations. If you require additional time, please let us know in advance.
- A $25 returned check fee applies to any payments that fail due to insufficient funds.
Communication & Availability
- We respond to voicemails and portal messages as promptly as possible, typically within 1 business day.
- We are not an emergency facility. If you are experiencing a crisis or medical emergency, call 911 or go to the nearest emergency room.
Social Media & Professional Boundaries
To protect your privacy and maintain ethical standards:
- We do not accept friend or contact requests on any social media platforms (e.g., Facebook, Instagram, LinkedIn).
- We will not discuss clinical matters via direct messages on social media.
Electronic Communication
- You may communicate with us via email, text, or the portal for administrative matters (e.g., scheduling).
- Do not use email or text for clinical issues or emergencies, as they are not secure for personal health information.
- If you prefer not to receive text/email communications, let us know and we will respect your preference.
Telehealth
We offer telehealth services when appropriate and legally permitted. These sessions are conducted using a HIPAA-compliant platform and carry the same confidentiality protections as in-person care.
- Telehealth is available for renewals only, not for initial medical marijuana certifications.
- Telehealth has certain limitations (e.g., inability to perform physical exams). Your provider may recommend an in-person visit if medically necessary.
Termination of Services
We value a respectful provider-patient relationship. Services may be terminated for any of the following reasons:
- The treatment is no longer clinically appropriate
- Repeated no-shows or non-payment for services
- Lack of participation or communication (e.g., 3 missed appointments without notice)
If services are discontinued, we are happy to provide referrals upon request.
Medical Marijuana Disclaimer
- MedLeaf Clinic does not sell, dispense, or distribute medical marijuana.
- We provide medical evaluations and physician certifications only, in accordance with Florida Statute § 381.986 and current OMMU regulations.
- We do not guarantee that a certification will be approved. The physician will evaluate eligibility and make decisions based on medical judgment and state law.
Payment Authorization and Refund Policy
- Payment is due in full at the time of service. We accept debit and credit cards only.
- All payments are final and non-refundable once the medical evaluation and/or certification service has been provided, regardless of outcome or patient decision to proceed further.
- By proceeding with services at MedLeaf Clinic, you agree not to dispute valid charges for services rendered unless there has been a billing error.
- You understand that medical marijuana remains federally illegal, and that certain financial protections (such as chargebacks) may not apply.
- If you initiate an invalid dispute or chargeback after services have been provided, MedLeaf Clinic reserves the right to recover the amount owed, plus any collection, administrative, or legal costs necessary to enforce this agreement.

By signing below or submitting this form electronically, I confirm that I have read, understood, and agreed to the above policies. I understand that failure to follow clinic policies may result in suspension or termination of services.
